We sought to characterize the relationship between a prosthetic ankle strain (PAS) and lower extremity muscle strength and mobility in older adults with a total knee arthroplasty (TKA). Thirteen older adults with a unilateral TKA were tested before and after partial weight bearing for the affected and unaffected limbs in a controlled, randomized order. Measurements included plantarflexor peak torque, isometric knee flexor and extensor peak torque, and gait velocity. PAS was measured using a universal goniometer. The strength of the PAS was calculated as the maximum passive dorsiflexion angle subtracted from the maximum passive plantarflexion angle. PAS significantly correlated with isometric knee flexor and extensor peak torque and gait velocity of the affected limb. When a cutoff of a PAS of 10° was used to discriminate between above- and below-normal isometric peak torque values, 69.2% of the participants had abnormal values for the affected limb (≥10°), and the PAS was a significant predictor of below-normal isometric knee flexor and extensor peak torque values.
